Serum T_(4) , T_(3) , T_(3)U , T_(4)/T_(3) and TSH in Chronic Renal Failure

Abstract

It had been well known that abnormal thyroid function tests and clinical thyroidal dysfunctional symptoms and signs were frequently seen in patients with chronic renal failure.
According to reports of other people, the level of T4 and TBG were usually normal, but the level of T3, T4/T3 and TSH were not constant. We measured total serum T4, T3, T3U, FT4 index, T4/T3 and TSH in 10 patients with chronic renal failure without nephrotic syndrome.
The results were as follows:
1) The serum T4, T3U and FT4 index were normal ranges in most patients. There was a tendency to lower T4 values as compared to normal control group, but this did not reach statistical significance(p>0.1).
2) The serum Ts was reduced markedly and T4/ T3 was elevated(p<0.01).
3) The serum TSH was decreased(p<0.01).
We concluded that the decreased level of T4 might be secondary to low serum albumin and low T3 values was due to reduced. Conversion T4 to T3 in extrathyroidal region.
The serum TSH was low, it could be presumed that the accumulation of abnormal metabolites or malnutrition play a role to reduce TSH secretion by pituitary &/or diminish TSH-responsiveness to thyrotropin releasing hormone.
